Telehealth: Health Information Technology

Our focus is to capture and improve the flow of updated client health information to and from each client and between providers.  The goal is to keep all parties best informed, as this is often where the ball gets dropped. Information gathered will be analyzed against national standards of outcome measures.  We generate report cards on client progress towards achieving care plan goals.
