Patient Centered Medical Home (PCMH)

We go beyond PCMH, into patient-driven health care.  We work with primary care physicians, especially those recognized as PCMH providers, to deliver outstanding care coordination services for their most complex patients.  Our activities and outcome measures will coincide with the PCMH requirements.
